Description	Percentage of children, 6 months - 20 years of age at the start of the measurement period, who have had tooth decay or cavities during the measurement period
Initial Patient Population	Children, 6 months - 20 years of age, with a clinical oral evaluation during the measurement period
Denominator	Equals Initial Population
Denominator Exclusions	Exclude patients who are in hospice care for any part of the measurement period.
Numerator	Children who had a diagnosis of cavities or decayed teeth in any part of the measurement period

Clinical Instructions

Track all patients 6 months to 20 years of age diagnosed with tooth decay or cavities.

Workflow

1. While documenting the Visit encounter, record the diagnosis, as appropriate
2. Open the Symptoms/Diagnosis section
3. Ensure the patient has a diagnosis of tooth or dental decay, as applicable
4. Continue documenting the encounter, as appropriate
5. When completed, Close and Archive the encounter
